In this section you will practise vocabulary from a medical record and exchanging information with a patient.

1. A patient record.

Use the words to complete the patient record.

 

Remember: First name = name, surname = last name
*Social security numbers are also used as an administration reference in many countries. For example, the National Insurance (NI) number in the UK.
Postcode (UK) = zip code (US)

2. Watch and listen.

A doctor is asking questions to complete the patient's record sheet. What does the doctor ask about? Listen and put the list in the correct order.

3. Listen and complete.

Listen again and choose the correct options to complete the patient record.

4. Complete the dialogue.

Use the words to complete the first part of the dialogue. (The exercise is in three parts.)

5. Questions.

Choose the correct word to complete each question.

6. Word order.

Put the words in order to complete the questions.

7. Medical abbreviations.

There are many abbreviations used in medical records. Match the abbreviations to the words.

10. Sentences.

11. Affirmative and negative sentences.

Look at the examples of affirmative and negative sentences.
